WHAT TO EXPECT

We give our patients the option to complete their new patient paperwork online before their appointment. Our office will send an email with the online paperwork to the address that was provided to the new patient coordinator when scheduling your initial appointment. This paperwork will be sent two days before your appointment date. If you have issues receiving the paperwork, please call our office at:talkdes404-256-2633.
If you do not wish to complete your paperwork beforehand, we recommend you arrive 30 minutes early to your appointment. Our front desk staff will give you an electronic tablet to complete your paperwork when you check-in.
You will be escorted to your room by one of our clinical assistants who work closely with each of our doctor’s teams. The clinical assistant will ask you a few questions about your medical history, medications, and symptoms. With this information, our physician’ assistant or doctor will be ready to discuss your condition, imaging, and care plan with you.
